EE System Required Documentation Elements

School districts and independent charter schools must maintain EE process records that demonstrate compliance with EE System requirements. The use of free DPI-developed forms will ensure and maintain compliance with all required element documentation for state model school districts and independent charter schools.

Principal EE Process Forms

We offer free, downloadable, growth-focused Educator Effectiveness (EE) System record-keeping files to support your local EE processes! These forms files are designed to streamline processes and information, support professional growth, provide flexibility in documentation based on local policies and needs of the individual educator, as well as capture the state-required documentation elements.

Features:

  • Identification of areas for growth or areas of strength ties together the self-review, goal-setting, and feedback processes
  • Professional Growth Portfolio displays information from all EE processes for a one-stop shop to review, reflect, and download for record-keeping.
  • Established focus areas for the year are displayed across forms to inform evidence collection and professional conversations.

Streamlined Record-Keeping

This version of EE forms is formatted with ALL of the Educator Effectiveness (EE) process forms combined into ONE FILE. This format has benefits:

  • One file to track, instead of a multitude of files. Tabs/Sheets can be printed or saved as PDF individually for record-keeping.
  • The ability to pull information and display information from one tab to another helps to streamline the processes/information. This may prevent some flipping between multiple files/forms to review information.

Google Sheets format

DPI can not provide the EE process forms in Google format at this time. However, previously downloaded Google-formatted files that utilize the 2022 Danielson Framework for Teaching may continue to be used for the 2026-27 school year. If interested in converting the Excel file below to Google Sheets, please contact DPIEducatorEffectiveness@dpi.wi.gov.
