- October 31, 2016:
- ED-1 - clarified the form should also be used for reevaluations
- ER-1 - clarified the determination of eligibility and need for special education)
- I-4 - clarified the IEP team must always describe how the student’s disability affects access, involvement and progress in the general education curriculum (see E.1. and 2.)
- I-7 suite - added hyperlinks to applicable assessment accommodations pages, reformatted district-wide form, and aligned language from assessment vendors
- May 6, 2016: Through a stakeholder process, the Sample Special Education Forms were revised to align with the new accountability system, Reading Drives Achievement: Success through Literacy. This new accountability system balances the monitoring of procedural compliance with improved student outcomes. New prompts on the Sample Forms encourage IEP teams to consider if a student has disability-related needs that affect reading and to develop goals, services and progress monitoring to address the needs and improve student outcomes.

- May 2, 2016: Added Spanish versions of I-7 District-Wide Assessment, I-7 DLM, and I-7 Forward to the all Spanish forms document.
- February 2016: The I-7 assessment forms have been updated. It is critical that IEP teams refer to the allowed accommodations and supports page for each assessment.
- I-7 Forward- Addition of new form for test administration in Spring 2016. Forward Exam accommodations and supports page.
- I-7 DLM- Updated to reflect the format of the DLM accommodations and supports.
- I-7 District-wide Assessment- Updated to remove the reference to statewide assessments.
- September 2015: The I-7 assessment forms have been updated to reflect the current accommodation and accessibility policies for Wisconsin assessments. IEP teams will notice the following changes:

- March 2014: IEP Model Forms I-7 Update
- IEP teams are responsible for deciding whether students with disabilities will participate in general education assessments with or without testing accommodations, or in the alternate assessment with or without accommodations. DPI model forms may be used to document these decisions. Forms in the assessment suite are to be completed based on which assessments the student will take during the implementation of the IEP. The forms are named, “I-7-[assessment].” For example, the form that IEP teams will use related to the Smarter Balanced Assessment (SBA) is I-7-SBA and contains language specific to SBA assessment policies.

- April 2009: New Form I-7-B, Guidelines for Oral Test Administration to Students with Visual Impairments on the Wisconsin Knowledge and Concepts Examination (WKCE) or the Wisconsin Alternate Assessment for Students with Disabilities (WAA-SwD)
- Beginning December 31, 2008, a parent of a child with a disability may revoke consent for the provision of special education and related services. The revocation of consent must be in writing. Upon receipt of the written revocation, the school district must promptly provide prior written notice in accordance with § 300.503 a reasonable time before stopping special education and related services.
The department has developed two model forms to assist districts in implementing this change in special education law. Form P-5 may be used to document written revocation of consent. Note, however, parents may use other ways of providing written revocation, and this form is not required. P-6 is a form districts may use to provide the required prior written notice. The department has also revised the Model Procedural Safeguards Notice and form P-1 (Determination and Notice of Placement: Consent for Initial Placement).- Memo on Revocation of Consent Requirements
