ELO First-time Application and Renewal Directions for School Audiologist

School Audiologist License Eligibility Requirements

Eligibility for the School Audiologist license requires an applicant to meet one of the two options below:

  1. The applicant meets all of the following requirements:

     

    • Completed 50 hours of a supervised practicum providing audiology services in a school setting, or equivalent experience while employed as a school audiologist by a public or private school or school district;
    • Possesses either a doctor of audiology degree or, if earned on or before August 31, 2018, a master’s degree or higher in audiology from an institution accredited by the education standards board of the American Speech−Language−Hearing Association;
    • Demonstrates knowledge and understanding of all of the following:
      • The pupil services standards cited under PI 34.004.
      • Educational psychology, including principles and theories of learning.
      • Methods and procedures in school audiology programs including the relationship with and content of school pupil services programs. This requirement may be met by prior experience upon the recommendation of the institution.
      • Alternative communication systems including signed language systems, their implications for the social, emotional, and educational development of children, and methods for effective communication with children who use them.
  2. The applicant holds a valid audiologist license issued by the Department of Safety and Professional Services.

How to Apply for First-time School Audiologist License

1. Determine if you must submit fingerprints

2. Gather Documents

Scan all documentation into files (.pdf, .doc/docx, .jpg/jpeg, or .txt) that are 2MB or less in size

  • Transcripts verifying degree information
    • PI-1612 and form filled out by audiologist program
  • DSPS Audiologist Certificate
  • Preview the ELO Conduct & Competency Questions to see if you will need to provide documentation of any alleged misconduct.

3. Log in to Educator Licensing Online (ELO)

  • Within ELO, make sure you have answered the three onboarding questions.
  • Find the Start a New Transaction for a New License section of the Quick Start Menu. Select the same choices in the drop-down menu in the application screenshots below:

O010 - 1020

  • Choose "4-Other Category"
  • Choose "G-School Audiologist License [O010]"
  • Choose "Request School Audiologist License [1020]"

 

O010 - 1015

  • Click the “GO!” button.
  • Complete each page of the application as instructed. Review your Application Summary and edit information if necessary OR click the “SUBMIT” button to submit your application.

4. Receive Email Confirmation

 

5. Complete Application on ELO

  • Click the GO! button next to “Submit C&C and Payment to Complete Application.”
  • Review your fees.
  • Complete the Conduct and Competency Questionnaire (14 questions).
  • Pay with a credit card or pre-paid card.

School Audiologist License Renewal Requirements

A School Audiologist license may be renewed if the licensee meets one of the following requirements:

  1. Obtains or maintains a valid license issued by the Department of Safety and Professional Services (DSPS).
  2. Obtains or maintains certification as an audiologist by the American Speech-Language-Hearing Association, American Academy of Audiology, or an affiliated credentialing board.
